Repair Authorization Form

• Non-Refundable Fee: A minimum diagnostic and evaluation fee of $75.00 is due upon instrument check-in.
• Repair Credit: If the customer authorizes the recommended repair scope within 14 calendar days of the diagnostic report, this $75.00 fee will be credited entirely toward the final labor total.
• Extensive Troubleshooting: Complex, intermittent faults, or deep historical research regarding vintage components will be billed at our master bench rate of $140.00/hour, applied after the initial evaluation period, with prior customer notification.

• In-House Stock: To maintain our rigorous quality standards and offer comprehensive warranties, this studio prioritizes parts sourced directly from our retail inventory.
• Customer-Supplied Parts: If the studio agrees to install a customer-supplied part, we offer no warranty on the component's functionality, longevity, or exact fit. Any additional labor required to modify a customer-supplied part to fit the instrument will be billed at the standard hourly bench rate of $140.00.

• Wood Movement: Instruments are made of organic materials responsive to temperature and relative humidity. Initial setups are calibrated to the shop's controlled environment. Subsequent wood movement due to seasonal shifts in the customer’s home is not a defect in workmanship.
• Inherent Risks: Certain procedures (including but not limited to: steam-separating dovetail neck joints, removing old acoustic bridges, or leveling frets on vintage instruments) carry inherent risks of finish chipping or wood fracture due to aged, brittle materials. The studio will use maximum master-level care but is not liable for unavoidable cosmetic aging anomalies revealed during structural work.

• Payment Terms: Full payment for labor and retail parts is due immediately upon completion of the repair, prior to the instrument leaving the premises.
• Storage Fees: Instruments not picked up within 30 calendar days of completion notification (unless pre-authorized) will incur a storage fee of $5.00 per day.
• Abandonment: Instruments left unclaimed for more than 60 calendar days from the initial completion notification may be considered legally abandoned. The studio reserves the right to sell the instrument to recover the costs of unpaid labor, storage fees, and retail parts used unless we are notified and reasonable arrangements are otherwise needed. This does not waive the daily storage fee.
